Yes, the Emergency Consent Health Services form needs to have the parent's signature notarized and be signed by a Florida doctor or nurse practitioner.
Submitting Health Forms
The Emergency Consent Health Services form needs to have the parent's signature notarized and be signed by a Florida doctor or nurse practitioner. This is one of the forms that must be completed and submitted before the start of the new school year. Once the form has been signed and notarized it is to be uploaded to the Magnus Health portal.
